FUNDING OPPORTUNITIES

The Foundation gives first priority to projects that increase community self-sufficiency and capacity. “Teaching people how to fish” will be given preference over providing fish. Building on Azusa’s existing community and strengths to create greater capacity for meeting community needs is the Foundation’s most strategic opportunity for funding.

FUNDING AVAILABILITY:

The estimated total of funds available for grants in 2009 will not exceed $_______. Applicants may apply for funds in amounts between $5,000 and $25,000. The Foundation encourages proposals whereby the requesting agency can participate in the cost sharing of the program with matching funds or other resource.


PROJECTS THAT ARE NOT ELIGIBLE FOR FUNDING:

  • Individuals for educational or other purposes
  • Political causes or candidates
  • Courtesy advertising
  • Religious, veteran or fraternal organizations, unless they are engaged in a significant program benefiting the entire community
  • Organizations seeking funding for building construction or other capital improvements

CALL TO PROPOSAL:

Proposals for grants must include the following information with respect to the principal organization.

Organization’s Profile

  • Organization’s name, address, phone number, email and website address
  • Tax identification number of the organization, if applicable
  • A listing of staff members and their qualifications
  • The organization’s contact person: title and contact information, including residential address
  • A listing of the board of directors with qualifications and home addresses
  • Identify the group(s) served
  • The amount requested

Organization’s Description and History

  • State the mission of the organization and its goals
  • Give an overview of the organization: when it was organized, who founded it, why was it started
  • Provide the following:
  • Information regarding the programmatic scope of the organization
  • A current itemized budget for the organization
  • A list of other grant sources and amounts obtained
  • A recent audited financial statement
  • A list of similar programs in which the organization has participated (This list should include a description of each project, the scope of its operations and budget, and its years of operation.)

New Program Description

  • Describe how this new program will create or increase the community’s self sufficiency and/or capacity
  • Provide the rationale for how this new program will complement your current programmatic offerings
  • Describe the population to be served by the new program
  • Identify the anticipated number of clients to be served
  • Describe the location of the program to be offered
  • A listing of entities in the partnership and their role in the program
  • Identify other sources of income or resources which the organization expects to receive for this program
  • Provide an itemized budget for the requested funds

New Program Evaluation

  • Provide a comprehensive explanation of the tools that will be used to measure the program’s success, include those that will measure the clients’ perception of service effectiveness
  • Provide a business plan with timeline and pro forma budget for this new program
  • Provide a timeline and business plan for replacing the funding provided by the foundation with alternative revenue sources which will ensure the ongoing expansion or visibility of the project
